Course book Form Downloads

Please note: these order forms are for Brown faculty, instructors & authorized affiliates only.

At present, these online order forms are processed through Microsoft Word, available on the Brown University Applications server. Please contact the Computer Information and Technology Department (CIT) for information on how to install Word on your computer.

If you already have Microsoft Word, please follow these simple instructions:

1. Download the Form document for Microsoft Word.
2. Fill out the form on your computer, using either the mouse or the keyboard (using the TAB key) to navigate from one field to another. The contact information in each form is required to process your order. Please do not forget to fill out any one of these fields.
3. After you've saved your changes, please attach the form to an email addressed to Textbooks@brown.edu
4. OTHERWISE: If you would like to use a blank form to fax to us or send via campus mail, download the blank printable forms.
